- Base Salary: $53.53 -$ 57.21 per hour +25% casual loading as per Classification RN 3.1- 3.3

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
ABOUT THE ROLE
The Registered Nurse – Aged Care will provide ongoing monitoring and communication with support workers to facilitate the prompt identification and response to clinical deterioration of elders receiving home care services, and will be a resource for the staff and provide a consultative approach with elders and families towards delivering care. This role is responsible for the completion of clinical assessments and comprehensive care-planning to ensure needs are met, in a person-centred approach with a focus on outcomes for each elder.
WHAT YOU WILL DO
The Registered Nurse –Aged Care is responsible for conducting aged care assessments and reviewing the care and services provided to elders to ensure that:
- Care and service plans are up-to-date and reflect the elder’s current needs, goals, and preferences.
- The Elder’s needs are met through the safe and effective delivery of care and services.
- Continuous updates are made to the care and services to integrate better practices as they become accessible
- Care and services are responsive to the changing needs, goals, aspirations, and choices of elders, communicating in appropriate formats to facilitate informed decision-making and choice.
SELECTION CRITERIA
Essential
- Current AHPRA registration as a Registered Nurse.
- Aged Care & Clinical Experience – Proven experience in aged care, community nursing, palliative care or chronic disease management.
- Identification of Clinical Deterioration – Strong ability to assess, respond to, and escalate changes in elder health conditions.
- Risk & Infection Control Management – Experience in managing high-impact risks, including falls, wound care, medication safety, and infection control.
- Cultural Competency – Understanding of Aboriginal and Torres Strait Islander culture with a commitment to culturally safe, person-centred care.
Desirable
- Aboriginal and/or Torres Strait Islander
- Ability to mentor and support junior staff
- Knowledge of Aged Care Quality Standards, clinical governance, and regulatory requirements in aged care
- Experience using Leecare or Communicare systems

- Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
- Current driver’s licence.
- Current CPR certificate or the preparedness to gain one within 6 months of commencement.
