About the roleThe Aboriginal Health Practitioner (AHP)/ Registered Nurse are responsible for identifying clients of concern clinically and follow appropriate and effective escalation procedures to ensure the right clinical response and minimise risk to clients.•You’ll IMPACT the community, helping close the gap in Indigenous healthcare and wellbeing, one helping hand at a time. •You’ll be PROUD, both of the work you do and who you work for. •You’ll work with a TEAM, alongside people who are down to earth and truly dedicated to what we do. •You’ll EXPERIENCE and learn something new every day through the variety of your role.•You’ll embrace the OPPORTUNITY to progress your career – follow your path at Danila Dilba. Responsibilities include: •Reviews clients in collaboration with the Indigenous Outreach Workers in direct contact with the client, following a risk-based schedule requiring:•Daily review for moderate risk clients•Inform clients of positive test results•Review on as required for low-risk clients should clients contact DDHS for support during the isolation period.•Escalates concerns to the Team Leader or the Senior Medical Officer based on clinical judgement•Responds quickly to concerns outside the schedule of reviews in consultation with the Team Leader.•Contributes to ongoing service improvement and designAre you the right person? The successful candidate will have: 1.Current registration as a Nurse / Aboriginal and/or Torres Strait Islander Health Practitioner with AHPRA.2.Demonstrated knowledge and experience working in a primary health care practice.3.Demonstrated experience working collaboratively within a multidisciplinary team4.Ability to work independently, and as a member of a team, with strong organisational skills and demonstrated ability to plan, manage and prioritise tasks.We want you to be proud of both the work you do and of who you work for!
