Date: Monday 1 and Tuesday 2 May 2023
Theme: Aboriginal Workforce and Chronic Care Models
The Aboriginal Chronic Conditions Network at the Agency for Clinical Innovation would like to invite authors to submit abstracts for the 2023 – Aboriginal Chronic Conditions Conference.
The conference will be held both virtually and in person at 1 Reserve Road, St Leonards, Sydney.
The aim of this conference is to showcase each other’s success and key learnings in designing and delivering chronic care models for Aboriginal peoples and communities.
A key focus of the 2023 conference is to:
• Highlight the critical importance that Aboriginal workforce play within chronic care models and improving health outcomes for Aboriginal peoples and communities.
• Showcase Aboriginal led innovations across Aboriginal Health and the chronic care space.
We call on abstracts that highlight the success or showcase innovations in Aboriginal workforce models and or Aboriginal chronic care. This includes across the Aboriginal Community Control
Health Services, Local Health Districts, Specialty Health Networks and other health related organisations across Australia.
