Clinical Nurse Specialist/ Chronic Nurse Navigator

The Nurse Navigator role is designed to provide holistic support to AMS registered patients with complex health conditions requiring an expert level of healthcare co-ordination. This integrated and proactive approach to health care service delivery will help ensure our patients receive the most appropriate service when and where they need it. Nurse Navigators work across system boundaries to actively advocate and improve health literacy, in close partnership with patients and families, other health and allied-health services to improve the patient’s healthcare experience and quality of lifeEssential:Outline your experiments with Chronic Care Management Health Care Plans and any outstanding outcomes.Current registration with the Nursing and Midwifery Board of Australia (AHPRA). 5 years or more experience.Ability to provide support to Aboriginal and Torres Strait Islander clients in a safe and culturally appropriate manner.Experience and knowledge with at least one or more chronic diseases.Ability to work effectively with GPs, practice nurses, Aboriginal Health Workers / Practitioners, allied health professionals and specialists.Demonstrated ability to work autonomously as well as a member of a multidisciplinary team.Strong understanding of Aboriginal Community Controlled Health Service, primary health care and Aboriginal and Torres Strait Islander issues within the Australian context.Strong interpersonal and written skills including liaison, negotiation and advocacy skills.Strong organisational skills and the ability to set priorities and achieved desired outcomes within specific timeframes.Knowledge to use a desktop computer, familiarity with Best Practices software.Possession of current driver’s licence.Applicants are required to have the right to work in Australia.
